A New Face in Cashiers

Advanced facial rejuvenation arrives in Cashiers as Dr. Harley opens a practice shaped by precision, warmth, and nearly two decades of expertise.

On the Highlands-Cashiers Plateau, where neighbors still stop to talk and word travels fast, a familiar name in Western North Carolina facial plastic surgery has put down new roots.

“I’ve been in practice for 18 years,” says David Harley, MD, “I started in Asheville and I have recently moved the practice to Cashiers.”

Dr. Harley is “a double board certified facial plastic surgeon,” with training that took him from Vanderbilt University Medical Center in Nashville to the Texas Medical Center in Houston, after graduating from Dartmouth College in Hanover, New Hampshire.

Today, he brings that depth of experience to a practice devoted to what he calls “facial rejuvenation, making [the] face more youthful and refreshed.”

His favorite – and most requested – approach pairs artistry with precision: “Deep plane facelift and deep neck lift surgery, combined with fat grafting and facial skin resurfacing.”

“I’ve had patients tell me they look 10 to 20 years younger after these procedures,” he says.

The practice also offers upper and lower blepharoplasty, brow and forehead lift, and lip lift, options designed to help features look rested, balanced, and naturally renewed.

In an area known for welcoming visitors, Dr. Harley’s practice has long been one that visitors seek out.

“We have been a destination practice since 2011 or 2012,” he explains, with patients traveling “from all over the country.”

Over the years, he has cared for people “from all 50 states and outside of the country,” something he calls “an incredible honor,” especially when patients are coming from major centers “such as New York City and Beverly Hills.”

Now, that same destination-level care is available close to home for the Plateau.

Another hallmark is how the surgeries are performed. “All the procedures are performed under local anesthesia with oral sedation,” Dr. Harley says. “In my practice, you do not need to have IV medication or a breathing tube to undergo these procedures.”

He also prioritizes a close physician-patient connection: “I always provide my patients with my personal cell phone number. I enjoy receiving text messages and photo updates from my patients as they heal.”

Always looking to enhance both outcomes and recovery, he has now brought new technology into the practice.

“We’ve added postoperative hyperbaric oxygen therapy,” he says, noting that the team has also installed a carbon dioxide laser for fractionated facial resurfacing.

To accommodate those additions, the practice is expanding at Old Cashiers Square, with offices at 25 Old Cashiers Square and 16 Old Cashiers Square.

That growth is anchored by a small team and a personal atmosphere.

“Our office is a small, close knit group,” Dr. Harley says.

Abigail manages the front desk and the administrative side of the practice, while Savannah assists with surgical procedures and postoperative appointments. And keeping every moving piece connected is the office manager, “and my wife,” Nicole, who, as Dr. Harley puts it, “does a little bit of everything!”

For the Harleys, this move is more than a new address – it’s home.

“We have lived and practiced in Western North Carolina for nearly 20 years,” he says, “but we are delighted to have found this hidden gem called Cashiers to settle down for good and raise our children.” With four children, “two boys and two girls age 7 to 13,” the family is building their future here, alongside a practice committed to helping patients look like themselves, just refreshed.
